To order raffle tickets online using your VISA or MASTERCARD DISCOVER or AMERICAN EXPRESS credit cards, please fill in the 'Order Form' below, then press the Submit button at the bottom of the form. That's all there is to it!

To order raffle tickets by FAX or mail, please complete the order form found at the bottom of this page, and print it.

Credit card orders can be FAXED to (513)308-0773.

For mail order, enclose a money order or check if not using a credit card, and mail it with a self-addressed, stamped envelope to:

If you have your computer configured with a FAX-compatible modem and appropriate software, you can have your computer FAX the order form to us without having to print it out.
To order in this fashion, fill out the 'Order Form' found on this page.
Next, print the 'Order Form' to your print queue that you have set up for sending FAX(es).
Fill in the phone number (513)308-0773 when your FAX software requests our FAX phone number. If you just have one phone-line/modem connection on your computer, you'll have to disconnect from your phone (internet) connection so that you can actually send in your order.
When ready, activate your FAX software so that your order can be FAXED to our Jazz Ceramics Procurement Department.
Orders are collected, maintain, verified and counted by the Raffle Judges. These orders are also vaulted for security until each drawing date.
Your information is hgih confidential and is not sold, traded, given, nor bartered to anyone else for any reason. You will be contacted by one of the Judges when you win.

VISA, MASTERCARD, AMERICAN EXPRESS and DISCOVER orders can be submitted via the Submit button at the bottom or FAXED to (513)308-0773.

Once your check or credit card charge has cleared, your ticket stubs will be snail-mailed and emailed to you prior to the drawing date. Ticket sales will commence on the 1st day of each month at 00:29 (12:29am), ticket sales will end on the last day of the month at 11:59 midnight, (one half hour later).
An announcement will be made when tickets have sold out or concluded.
